29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.    WORKSHOP:  The employer had not established and implemented step-by-step hazardous energy control lockout/tagout procedures in the machine specific for the employees during servicing and maintenance of equipment such as (but not limited to) the Line 1 pack down machine.      On/or about August 30, 2017, an employee was performing cleaning/blow down on the rotary airlock after a product changeover on Line 1 (Machine) while the Machine was still energized, and the employee's fingers came in contact with the rotary airlock resulting in severe finger amputations.        Electric Cable Compound, Inc. was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA Inspection Number 964208, Citation Number 1 and Item Number 1a, issued on 05-06-2014, with a final order date of 05-29-2014 , with respect to a workplace location at 108 Rado Drive, Naugatuck, CT.

29 CFR 1910.147(c)(7)(iii)(B): Additional retraining shall also be conducted whenever a periodic inspection under paragraph (c)(6) of this section reveals, or whenever the employer has reason to believe that there are deviations from or inadequacies in the employee's knowledge or use of the energy control procedures.    WORKSHOP:  The employees, as authorized, affected, and/or other types were not provided with re-training and information in the controlling of potentially hazardous and unexpected releases of energy sources (lo/to) since the incident happened on/or about August 30, 2017 whereby an employee sustained amputations to the tips of fingers while performing cleaning tanks on the Line 1 machine.    Electric Cable Compound, Inc. was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1910.147(c)(7)(iii)(B), which was contained in OSHA Inspection Number 964208, Citation Number 1 and Item Number 1c, issued on 05-06-2014, with a final order date of 05-29-2014 , with respect to a workplace location at 108 Rado Drive, Naugatuck, CT.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.    ESTABLISHMENT:  The in-patient hospitalization of an employee who sustained severe fingers amputations while cleaning/blowing down a rotary airlock on Line 1 machine was not reported to OSHA within 24 hours.

When an authorized government representative asks for the records you keep under Part 1904, you must provide copies of the records within four (4) business hours.    ESTABLISHMENT:  The OSHA 300 log of Work-Related Injuries and Illnesses for the year of 2015 and the OSHA 301 Incident Report were not submitted the OSHA representative within four (4) business hours.
