29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    ESTABLISHMENT: The employer did not ensure that employees were not only provided with but also wearing face shields during the tasks, such as (but not limited to ) cleaning/purging process of an extruder machine.    On or about 06/24/2019, pieces of hot plastic spewed from the extruder while two (2) employees were performing the above task, and one of employees not wearing a face shield was struck, sustaining severe burns to the left side of face, ear, and right wrist.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    ESTABLISHMENT: The two (2) flexible metallic conduits containing flexible cables connected to the Die Heat electric box that was supplying power to the line #1 extruder machine were separated from the fittings.
