29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: Facility: 6-foot fork extensions were being used on the forks of theYale Lift Truck Model GLC060TGNUAE084 (S/N E187V22590B) to move oversize pallets of sheet metal. No written approval from the forklift manufacturer was obtained by the employer. The addition of fork extensions decreases the capacity of the forklift.
