29 CFR 1910.179(b)(8): Employee(s) other than designated personnel were permitted to operate crane(s):  a. In the steel shop, where bridge and jib cranes were used for moving trailer parts and jigs, employees that had not been designated (authorized) had operated cranes.  (Note: in addition, employees that had been authorized did not know they were authorized, employees reported that they had not received on the job training, and employees reported that other employees, other than supervisors or team leaders, had directed them to operate cranes under their observation.  The employer did not have authorization documentation on at least one long term operator.)

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employee(s) were exposed to the hazard of being crushed:  a. In the steel shop, where bridge cranes were used for moving trailer parts, trailers, and jigs, contact was made when one crane was run into another which could and did result in an employee being struck by a load supported by a crane.  One feasible  and acceptable abatement method, which was selected by the employer, is to install anti-collision sensors.
