Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards related to ladder usage: a) Manac Trailers USA, Inc. @ 8593 Highway 77, Oran, MO 63771: On and before November 17, 2014, employees utilized Werner fiberglass step-ladders in the steel shop. An employee used a 12 foot step ladder that was folded and leaned against a trailer, so that the ladder base section was not mounted on secure footing. This condition exposed the employee to a fall hazard of approximately 8 feet. b) Manac Trailers USA, Inc. @ 8593 Highway 77, Oran, MO 63771: On and before November 17, 2014, employees utilized Werner fiberglass step-ladders in the steel shop. An employee used a 12 foot step ladder that was positioned away from the work area, which required the employee to reach to his right and so his body was no longer centered between the side rails of the ladder. This condition exposed the employee to a fall hazard of approximately 8 feet.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards related to the use of an unrated below-the-hook lifting device: a) Manac Trailers USA, Inc. @ 8593 Highway 77, Oran, MO 63771: On and before November 25, 2014, employees in the Steel Sub Assembly area were exposed to struck-by hazards in that an unrated, below-the-hook lifting device. Employees used a below-the-hook lifting device, a coupler clamp, to lift fabricated couplers, estimated at 600 pounds, with an overhead crane. The capacity of the in-house, designed and fabricated below-the-hook lifting clamp was unknown.

29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Manac Trailers USA, Inc. @ 8593 Highway 77, Oran, MO 63771: On and before November 25, 2014, the employer did not ensure that employees in the Top Rail Beam Staging area were protected from amputation hazards. The KBC Machinery BAH-916 horizontal band saw was not equipped with a guard to prevent against accidental contact. The saw blade was unguarded approximately six inches past the point of operation. This condition exposed employees to amputation hazards.

29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not protected from the rays by noncombustible or flameproof screens or shields: a) Manac Trailers USA, Inc. @ 8593 Highway 77, Oran, MO 63771: On and before November 17, 2014, the employer did not have welding screens to define welding areas in the Aluminum Line Shop. Two welders utilized EASB MIG 4002CW welders to weld aluminum side rails in a location that could have been walled off by welding screens to protect other employees in the area. This condition exposed employees in the aluminum line shop to arc flash hazards.

29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels: a) Manac Trailers USA, Inc. @ 8593 Highway 77, Oran, MO 63771: On and before November 25, 2014, the employer did not ensure that an employee on the aluminum line was provided an annual audiogram. The employee, an aluminum welder in the side rail welding area, was exposed to a continuous noise level at 83.5% of the allowable 8-hour time-weighted average sound level (85 dBA). The equivalent dBA level of 83.5% is approximately 88.7 dBA. The sampling was performed for 431 minutes.
