29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a)  Warehouse/ TLM, Realty Office;  A refrigerator, Proctor Silex toaster, Hamilton Beach kettle and Black and Decker coffee  pot were plugged into a relocatable power tap (RPT). The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.  This exposes employees to possible electrical fire hazards.   (b)  Warehouse/ TLM, Realty Office, cubicle by break area;  A black RPT was plugged inot a white RPT which powered computer equipment. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords.  This exposes employees to possible electrical fire hazards.   (c)  District Office cubicle:  A Holmes heater was plugged into a black RPT. The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.  This exposes employees to possible electrical fire hazards.   (d)  Untanking Tower, Submaintenance computer area;  A computer was plugged into an orange extension cord, violating the extension cords UL listing and labeling for temporary power.  (e)  Untanking Tower, Submaintenance computer area;  A white relocatable power tap with a light was plugged into a yellow and black extension cord.  The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords.  This exposes employees to possible electrical fire hazards.   (f)  Untanking Tower, Submaintenance computer area;  A white relocatable power tap with a light was re-wired into a baloney cord and the device was not tested for intrinsic electrical safety.  This exposes employees to possible electrical fire hazards.   (g)  Untanking Tower, Environmental Office;  A Lasco heater was plugged into a relocatable power tap. The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.  This exposes employees to possible electrical fire hazards.      Bonneville Power Administration was previously cited for a violation of this standard which was contained in OSHA inspection no. 908523 citation 1, item 1, issued on September 27, 2013 with respect to a workplace at Vancouver, Washington.     Bonneville Power Administration was previously cited for a violation of this standard which was contained in OSHA inspection no. 542122 citation 1, item 4, issued on October 2, 2012 with respect to a workplace at Olympia, Washington.
