29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    On or about April 8, 2014 and times prior thereto, employees of Anthony Oak Flooring, Inc. operated powered industrial vehicles inside and outside of the employer's facility located at or near 409 North Vine Street in Magnolia, Arkansas.  The employer did not require employees operating vehicles to wear seatbelts as required by the manufacturer.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    On or about April 8, 2014 and times prior thereto, employees of Anthony Oak Flooring, Inc. operated forklifts inside and outside the employer's facility located at or near 409 N. Vine Street in Magnolia, Arkansas.  The employer had not ensured each operator was competent to operate a powered industrial truck safely by successful completion of the training required, exposing employees to tip-over, struck-by, and crushed hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about April 8, 2014 and times prior thereto, employees of Anthony Oak Flooring, Inc. operated woodworking machinery for the manufacture of wood flooring in the employer's facility located at or near 409 North Vine Street in Magnolia, Arkansas.  A Hasko MatchMaster with model number FSM and serial number 9921405 was used by employees for the placement of tongues and grooves into sides of the wood.  Guarding was not provided in three (3) locations on the Hasko MatchMaster where wood was fed into the machine, exposing employees to rotating parts.

29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft:    On or about April 9, 2014 and times prior thereto, employees of Anthony Oak Flooring, Inc. operated woodworking machinery for the manufacture of wood flooring in the employer's facility located at or near 409 North Vine Street in Magnolia, Arkansas.  The following instances of unguarded projected shaft ends were found in the facility exposing employees to caught-in hazards:    a. An unguarded projecting shaft end was observed on the Froedge conveyor system with model label Landing Deck and serial number 41404.  The projecting shaft end was located 56 inches from the ground and protruded from the conveyor frame by 3 inches with a 1 inch diameter.      b. Two unguarded projecting shaft ends were observed on operators station near the Froedge conveyor system with model label Landing Deck and serial number 41404.  The projecting shaft ends each protruded 3 inches from the conveyor frame with a 1 inch diameter.    c. An unguarded protruding shaft end was observed on the operators station near the Froedge conveyor system with model label Landing Deck and serial number 41404.  The projecting shaft end was located 45 inches from the ground and protruded 3 inches from the conveyor frame with a 2 inch diameter.      d. An unguarded protruding shaft end was observed in the upper rip saw area near the operator's station at the conveyor system feeding from the Hasko SR-30 Gang ripsaw.  The protruding shaft end protruded 4 inches from the conveyor frame with a 2 inch diameter.    e. An unguarded protruding shaft end was observed in the upper rip saw area near the operator's station at the conveyor system feeding from the Hasko SR-30 Gang ripsaw.  The protruding shaft end was located 18 inches from the ground and protruded 2 inches from the conveyor frame with a 1 inch diameter.    f.  An unguarded protruding shaft end was observed on the Froedge conveyor with model 50' Rooftop Deck and serial number 41271.  The projecting shaft end was located 30 inches off the ground and protruded 2 inches from the frame of the conveyor with a 1 inch diameter.    g. An unguarded protruding shaft end was observed on the Froedge conveyor with model 50' Rooftop Deck and serial number 41271.  The projecting shaft end was located 32 inches off the ground and protruded 2 inches from the conveyor frame with a 1 inch diameter.
