29 CFR 1910.305(a)(2)(x): Flexible cords and cables shall be protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points.   a.  At the work site, employees operated metal saws with movable heads to cut a specific length and angle of aluminum and steel framing pieces for assembling signs.  A flexible electric cord for an overhead light fixture was run where it was repeatedly struck by the movable metal saw head assembly.  This caused damage to the insulation around the cord and tape had been wrapped around the damaged area.

29 CFR 1910.305(g)(1)(iv): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used:  29 CFR 1910.305(g)(1)(iv)(A): As a substitute for the fixed wiring of a structure;  a.  At the work site, above a Mepal Italia movable head metal saw, there was a flexible cord used as permanent wiring to supply power to an overhead fluorescent light fixture, exposing employees to electrical shock and fire hazards.  b.  At the work site, employees operated a Grizzly drill press that was connected to a flexible cord hanging from the ceiling that was used as permanent wiring, exposing the employees to electrical shock and fire hazards.  c.  At the work site, above an Italia movable head metal saw, there was a flexible cord used as permanent wiring to supply power to an overhead fluorescent light fixture, exposing employees to electrical shock and fire hazards.

29 CFR 1910.303(h)(3): Work space about equipment. Sufficient space shall be provided and maintained about electric equipment to permit ready and safe operation and maintenance of such equipment. Where energized parts are exposed, the minimum clear work space may not be less than 1.98 m (6.5 ft) high (measured vertically from the floor or platform) or less than 914 mm (3.0 ft) wide (measured parallel to the equipment). The depth shall be as required in paragraph (h)(5)(i) of this section. In all cases, the work space shall be adequate to permit at least a 90-degree opening of doors or hinged panels.  a.  At the work site, at the time of inspection, an electrical branch circuit box behind the air compressor in the CO2 laser area was blocked by equipment in the area that included (but was not limited to) light curtains for the laser, the air compressor and the dust collection unit.

29 CFR 1910.303(f)(2): Services, feeders, and branch circuits. Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident.  a.  At the work site, an electrical branch circuit box was located between the Bystar 3015 laser cutter and the wall.  The electrical branch circuit box was not externally labeled to indicate the machine/equipment to which it was connected.  b.  At the work site, an electrical branch circuit box was located in the wall behind the Italia adjustable head aluminum saw.  The electrical branch circuit box was not externally labeled to indicate the machine/equipment to which it was connected.
