29 CFR 1910.106(e)(6)(ii): Grounding. Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), shall not be dispensed into containers unless the nozzle and container are electrically interconnected. Where the metallic floorplate on which the container stands while filling is electrically connected to the fill stem or where the fill stem is bonded to the container during filling operations by means of a bond wire, the provisions of this section shall be deemed to have been complied with.   a. On or about July 26, 2017, located in the flammable storage room, employees dispensed Kardol Acrylic Lacquer Thinner with a flashpoint of -4 degrees Fahrenheit from 55-gallon drums into plastic containers without the containers being grounded and bonded, thereby exposing the employees to an explosion/fire hazard.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.   a. On or about July 26, 2017, employees operated a table router that did not have any means of guarding to protect employees from the flying aluminum or polycarbonate chips, thereby exposing the employees eyes to struck-by hazard.

Compressed air shall not be used for cleaning purposes except where reduced to less than 30 p.s.i. and then only with effective chip guarding and personal protective equipment.   a. On or about July 26, 2017, located at the router table employees used 89 p.s.i. of air to clean chips off the table and floor, thereby exposing the employees to being struck-by flying chips and/or air embolism.

29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed.     a. On or about July 26, 2017, located in the fabrication shop, there was an 240 volt, 3 phase electrical disconnect  that had an unused opening in the side of the disconnect box, thereby exposing employees to electrical shock and/or arc flash hazards.        b. On or about July 26, 2017,located in the fabrication shop, there was a 480 vac electrical panel along a wall that had an unused opening in the side of the panel box, thereby exposing employees to electrical shock and/or arc flash hazards.        c. On or about July 26, 2017, located in the back of the shipping department, there was a 480 vac electrical panel that had an unused opening in the upper side of the panel box, thereby exposing employees to electrical shock and/or arc flash hazards.

29 CFR 1910.305(d)(2): Panelboard enclosures. Panelboards shall be mounted in cabinets, cutout boxes, or enclosures designed for the purpose and shall be dead front. However, panelboards other than the dead front externally-operable type are permitted where accessible only to qualified persons.       a. On or about July 26, 2017, there was a 480 electrical panel that was used in part to supply electrical power to a buss bar, Miller and Hobart welder, shop heater, etc. The electrical panel had a piece of polycarbonate as a means of a dead front, thereby exposing the employees to an arc flash incident.
