29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  a.  Examples existed of damaged power cords and extension cords where there was insulation damage at the plug (no strain relief), such as with a jig saw, extension cord used to power the jig saw, and a belt sander.  Abatement documentation is required for this violation.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  a.  An extension cord with a missing ground pin was used to power a hand tool; there was no visual inspection to ensure that electrically powered equipment and cords were not damaged before use.  (Note: according to the standard visual inspection is not required when equipment is left in place where there has been no change to the equipment, however, before its use the equipment must be inspected to ensure it is not damaged.)  Abatement documentation is required for this violation.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a.  An annual inspection of the lockout program was not made and documented, including inspection of written specific procedures, ensuring that employees have been trained and only authorized employees are performing lockout (except for cord and plug machines where the machine is unplugged before servicing and the plug is within site of the employee), and lockout is conducted according to the requirements of the standard, such as placing a lock on the designated energy isolating device and subsequent applicable testing of the machine to ensure it is locked out.  (Note: lockout is not to be confused with machine guarding, and where guarding is adjusted correctly so that exposure is controlled according to the design of the guard.)  Abatement documentation is required for this violation.

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section:  a.  Specific lockout procedures were not in writing according to the standard requirements (see 1910.147(c)(4)(ii)(A) - (D)), which included the specific type of energy (such as electrical, air and the quantity, such as amount of voltage and pressure), shut down procedures, where to place the lock and which energy isolating device to use, and the specific method to test the machine to ensure energy has been isolated.  Abatement documentation is required for this violation.
