29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    JOB SITE: 1012 Industrial Park Drive, Montgomery City, MO 63361, on or about 4/6/2018, an extension cord with a missing ground pin was used to power portable power tools.  There was not visual inspection of the cord for external defects (missing pins, or damage to outer jacket or insulation).  The workers were exposed to electrical hazards.    BRANDKAMP INDUSTRIES, INC., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CRR 1910.334(a)(2)(I), which was contained in OSHA inspection number 967745, citation number 1, item number 1c, issued on 4/25/2014, and was affirmed as a final order on 6/5/2018 with respect to a workplace located at 1012 Industrial Park Drive, Montgomery City, MO 63361.
