29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  The employer did not train his employees on the hazards of falls before being assigned the task of commercial roofing operations.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c)

29 CFR 1926.501(b)(4)(iii): Each employee on a walking/working surface was not protected from objects falling through holes (including skylights) by covers:    At a jobsite in Rockford, Illinois, skylights located on a flat commerical roof approximately 26 feet above grade were not provided with protective covers or guardrails to protect workers from falls.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    At a jobsite in Rockford, Illinois, employee working approximately 26 feet above grade on a flat commercial roof were not using any method of fall protection.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)

At a jobsite in Rockford, Illinois, employees were using a walkway bridging 2 roofing systems approximately 26 feet above grade was not equipped with midrails.
