Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   a.  On or about August 26, 2016, at the above addressed jobsite, employees performing roofing operations on a low sloped roof (4:12 or less), twenty five feet (25) six (6) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.
