29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, was not protected by protective helmets.     a) Front and side of residence located at 115 North Hill Brook Trail. The employees working directly below the re-roofing operations were not wearing protective helmets, hard hats, to prevent injury from being struck by either falling equipment or material.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) Roof of residence located at 115 North Hill Brook trail, Johns Creek, Georgia. Employees installing asphalt shingles while working from an unprotected steep-sloped roof were exposed to fall hazards of approximately 21-feet 2-inches.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.     a) Roof and side of residence located at 115 North Hill Brook trail, Johns Creek, Georgia. Employees were exposed to fall hazards while mounting and dismounting an aluminum extension ladder. The side rails of the ladder did not extend at least 3-feet above the upper landing, a steep pitched, 10 in 12, unprotected roof, which was 21-feet 2-inches above ground level.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not marked in a manner that readily identified them as defective, tagged with "Do Not Use" or similar language, or withdrawn from service until repaired.   a) Front side of residence located at 115 North Hill Brook Trace, Johns Creek Georgia. Employees were exposed to fall hazards of 21-feet 3-inches as an aluminum extension ladder, used to access the roof, was damaged. One of the no skid feet was damaged while the other foot was completely missing.
