1926.502(d)(16)(iii): Personal fall arrest systems, was not rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level: a) On or about April 22, 2014, at the above located worksite, two employees were performing roofing operations approximately 10 feet above the ground while wearing personal fall arrest systems that were not rigged to prevent a free fall more than 6 feet (1.8 m), and/or allow contact with the ground, thereby exposing the employees to the hazards associated with falls. b) On or about April 22, 2014, at the above located worksite, an employee was working on a steep pitch (16/12 pitch) roof approximately 30 feet above the ground while wearing a personal fall arrest system that was not rigged to prevent a free fall more than 6 feet (1.8 m), and/or allow contact with the ground, thereby exposing the employee to the hazards associated with falls.

29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about April 22, 2014, at the above located worksite, an employee working on a steep pitched (16/12 pitch) roof approximately 20 feet above the ground was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. The Total Roofing & Construction Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 312599343 citation number 1 item number 2 and was affirmed as a final order on January 21, 2010 with respect to a workplace located at 150 E. 170th Street, Fire Station #2, South Holland, IL. 60473.
