29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zone(s) during operating cycle(s): 1) On April 23, 2014, and at prior times, the shear, angle shear, and bender on the Dvorak Machine Co. ironworker were not equipped with a point of operation guard, device, awareness barrier, or adjustable restricter. 2) On April 23, 2014, and at prior times, the shear, angle shear, and round stock cutter on the Upton Bradeen James ironworker were not equipped with a point of operation guard, device, awareness barrier, or adjustable restricter. Note: Abatement can be provided by complying with the requirements of ANSI B11.5, Safety Requirements for the Construction, Care, and Use of Iron Workers.
