29 CFR 1910.134(c)(1):A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: (a) 4836 Van Gordon Street, Wheat Ridge CO 80033: On or about April 7, 2021, and at times prior, the employer did not establish a written respiratory protection program for use of abrasive blasting respirators in the workplace. Employees are exposed to Particulates Not Otherwise Regulated while performing abrasive blasting duties using coal slag blasting media. Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1910.134(e)(1):The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: (a) 4836 Van Gordon Street, Wheat Ridge CO 80033: On or about April 7, 2021, and at times prior, the employer did not provide a medical evaluation to employees who were required to wear abrasive blasting respirators. Employees use abrasive blasting respirators when conducting blasting operations using coal slag. Due to lack of medical evaluations prior to wear of the respirator, employees may be exposed to other hazards such as aggravation of underlying medical conditions including but not limited to asthma or claustrophobia. Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet). Note: The employer may discontinue an employee's medical evaluations when the employee is no longer required to use a respirator.
