On or about April 23, 2014, place(s) of employment were not kept clean:  a) PTO#1, #2, and #3, where dust collects on the duct work and where there have been fires inside the ovens the employees are exposed to an explosion/fire hazard from the dust that that has collected on the ductwork, and ledges above the finished floor.

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section.    On or about April 23, 2014 at 14187 Nisshinbo Drive, Covington, GA: Employees were exposed to sudden energization or start-up of multi-energy source. During the programmatic review of the lockout/tag out machine/equipment specific procedures to include, but not limited to the CL04 Grinding Loader, CL04 Grinder,CL04 Scorching Loader, CL04 Scorching Unit, CL04 Cooling Tower, Cl04 Unloader, CL04 Shot Blast Loader, CL04 Shot Blast, CL04 Powder Coat, CL04 Powder Coat Loading Robot and similar machine it was revealed that there were deficiencies in the employers lockout/tag out specific procedures.  The employer has identified the machines energy sources as pneumatic and electrical. The deficiencies are the following:      a) Procedures did not include a specific statement of the intended use of the procedures.    b)Specific procedural steps for shutting down, isolating, blocking and securing machine or equipment to control hazardous energy were not identified and/or listed on the machine specific procedures.     c) Specific requirements for testing the machine or equipment to determine and verify the effectiveness of lockout devices, tag out devices and other energy control measures.     d) Specific procedures did not allow for notification of affected employees prior to the controls being applied and after they are removed.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s).  On or about April 23, 2014 on line AL04 and AL07 were exposed to a caught by hazard:  a)  on the Shim Bond #1 where brakes are labeled, employees are exposed to a caught by hazard when the two handed trip could be operated with one hand.  b) an employee working on AL04 was exposed to a caught by hazard when the Plexiglas guard was missing on the Mercedes brake machine.
