29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet (1.22 m) or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: a) On or about April 15, 2014, located in Building 2 an employee checked the Benz Oil level(s) in a 250 gallon tank that was located on the mezzanine. The tank was set on three wooden pallets which increased the employee working height to 1.5 feet above the standard guardrail system exposing the employee to a 13.5 foot fall to the concrete floor below. No other alternative conventional or personal fall protection system(s) were provided to do that work.
