29 CFR 1910.178(p)(1):If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition. On or about August 16, 2023 at location 9001 Business Hwy 51, Rothschild, Wisconsin: employees were operating a Toyota (Model: 05-8FBM25T Serial: 05-8FBM25T-10123) forklift with damage including, but not limited to, chunks of tire missing, bald tires, and splitting on the side wall which could result in the tires to lose traction and possibly expose forklift operators to tip-over hazards.
