29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. (a) Front side, new residential construction located at 800 Stuart Drive, Alpharetta, GA 30004. Employee was exposed to falls of approximately 15-feet while installing cornice boards and siding from a ladder jack scaffold platform without a fall protection system in place. Employee was wearing a full body harness with no connection to the life-line provided.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. (a) Ladder jack scaffold platform on front side, new residential construction located at 800 Stuart Drive, Alpharetta, GA 30004. Employee was exposed to eye injuries while operating a Hitachi (Model NR83A2) Framing Nail Gun and a Hitachi (Model NT65M2-S) 2-1/2-inch Finish Nail Gun without safety glasses. The employee was exposed to fasteners and flying debris while operating these pneumatic tools at 120 PSI without eye protection. Antonio Romero was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 687278, citation number 1 , item number 1 and was affirmed as a final order on 11/19/2012, with respect to a workplace located at 4201 Hardy Avenue, Smyrna, GA 30080.
