29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.    a) The employer failed to provide documentation and certification for abatement of citation 1 item 1 in which a competent person did not conduct frequent and regular inspections of the jobsite. Condition noted on or about 05/01/14.    b) The employer failed to provide documentation and certification for abatement of citation 1 item 3 in which employees were exposed to fall hazards in excess of 11 feet at unprotected, elevated edges without the use of any fall protection. Condition observed on or about 05/01/14.    c) The employer failed to provide documentation and certification for abatement of citation 1 item 4 in which employees had not received any fall protection training. Condition noted on or about 05/01/14.    d) The employer failed to provide documentation and certification for abatement of citation 1 item 5 in which employees were exposed to fall hazards in excess of 10 feet when the ladder's side rails did not extend at least 3 feet above the landing. Condition observed on or about 05/01/14.    e) The employer failed to provide documentation and certification for abatement of citation 1 item 6 in which employees were exposed to visible ladder defects such as damaged rungs, missing feet, missing rope, ect. Condition observed on or about 05/01/14.    f) The employer failed to provide documentation and certification for abatement of citation 1 item 7 in which employees did not received any ladder hazard awareness training.
