29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe: (a) Work bench on south side, new residential construction located at 3708 Thackary Drive, Powder Springs, GA 30127: Employee was exposed to severe laceration and amputation hazards while using two Makita (Model-5007F) 7-1/4-inch Circular Saws with the lower guards propped open with a nail. A nail was used between the upper guard and the lower guard to prevent the lower guard from retracting after a cut through the siding material was made.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end. (a) South side, new residential construction located at 3708 Thackary Drive, Powder Springs, GA 30127: Employee was exposed to fall hazards while working on a 12-inch x 24-foot aluminum work plank at heights of approximately 19-feet, 5-inches with the work platform extended at least 10-feet past the support Alum-A-Pole without securing the cantilevered end as no guardrails were used to prevent employees from accessing the extended portion of the working surface.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. (a) South side, new residential construction located at 3708 Thackary Drive, Powder Springs, GA 30127: Employees were exposed to fall of approximately 11-feet, 3-inches to 19-feet, 5-inches while installing Hardie-Plank Siding from a pump jack scaffold with no fall protection system in place. Guardrails were not installed along all open ends of the work platform and personal fall arrest systems were not provided to prevent falls to the lower level.
