29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    (a) Rear side, new residential construction located at 2709 Canopy Circle, Alpharetta, GA 30009: Employee operating Makita 7-1/4-inch circular saws and Hitachi pneumatic finish and siding nail guns were exposed to eye injuries while operating these tools without protective eyewear.    Emmanuel Construction Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1018774, citation number 1, item number 1 and was affirmed as a final order on 03/23/2015, with respect to a workplace located at 3005 Evelyn Lane, Milton, GA 30004.

29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:    (a) Rear side, new residential construction located at 2709 Canopy Circle, Alpharetta, GA 30009: Employees were exposed to severe laceration and amputation hazards while using a Makita (Model - 5007F) 7-1/4-inch Circular Saw with the lower guard propped open with a wood shim. A shim was used between the upper guard and the lower guard to prevent the lower guard from retracting after a cut through the siding material was made.     Emmanuel Construction Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.304(d), which was contained in OSHA inspection number 975601, citation number 1, item number 1 and was affirmed as a final order on 07/08/2014, with respect to a workplace located at 3708 Thackary Drive, Powder Springs, GA 30127.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    (a) Rear side, new residential construction located at 2709 Canopy Circle, Alpharetta, GA 30009: Employees were exposed to falls of approximately 20-feet, 5-inches while installing Hardie-Shake Siding from a pump jack scaffold with no fall protection system in place. Guardrails were not installed along all open ends of the work platform and personal fall arrest systems were not provided to prevent falls to the lower level.     Emmanuel Construction Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 451(g)(1), which was contained in OSHA inspection number 975601, citation number 1, item number 3 and was affirmed as a final order on 07/08/2014, with respect to a workplace located at 3708 Thackary Drive, Powder Springs, GA 30127.    Emmanuel Construction Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 451(g)(1), which was contained in OSHA inspection number 1018774, citation number 2, item number 1 and was affirmed as a final order on 03/23/2015, with respect to a workplace located at 3005 Evelyn Lane, Milton, GA 30004.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  (a) East side, new residential construction located at 2709 Canopy Circle, Alpharetta, GA 30009: The employer failed to remove a damaged 100 ft. extension cord from service as the protective casing was damaged, frayed and exposing internal insulated wiring. The extension cord had a damaged strain relief and a missing ground pin as this damaged cord was providing power to a Makita (Model - 5007F) 7-1/4-inch circular saw from a temporary GFCI power source.
