29 CFR 1910.106(e)(6)(ii): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), were dispensed into containers without the nozzle and container being electrically interconnected.  Employees dispensed Acetone and other flammable liquids from large metal drums into metal Justrite containers for use in the production area and the dispensing nozzle and containers were not electrically interconnected during dispensing.

29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of Particulates Not Otherwise Regulated - Respirable Fraction listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 5mg/m3.  A sanding employee in the sanding area on 06/17/2014 was exposed to Particulates Not Otherwise Regulated - Respirable Fraction at an 8-hr time-weighted average level of 6.1 mg/m3, approximately 1.2 times the limit of 5 mg/m3; this limit established to prevent respiratory illness and other illnesses. The exposure level is derived from two samples collected over a 409 minute period.

29 CFR 1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d):  Employees engaged in sanding of cultured marble products was exposed to Particulates Not Otherwise Regulated - Respirable Fraction in excess of the permissible exposure limit and all feasible administrative and/or engineering controls were not implemented.  General methods of control applicable in these circumstances include, but are not limited to, the following:       a) Increased ventilation in the sanding hut area including local exhaust ventilation.         b) An employee rotation program to reduce time of exposure.       c) Modification of the water system to reduce dust.
