29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. On or about April 28, 2015, an employee underwent the surgical amputation of the tip of the right index finger as a result of a work-related incident that occurred that same day and the employer failed to report to OSHA of the work-related amputation within twenty-four (24) hours.
