29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting:          Carstin Brands, Inc. had portable fire extinguishers within their facility for employee use in the event of a fire.  The company did not provide an educational program to employees, in order to familiarize employees with the general principles of fire extinguisher use and the hazards associated with incipient stage firefighting.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:        Carstin Brands, Inc. had employees perform maintenance operations on equipment including but not limited to: saws, cnc machines, and sanders, and the employer did not develop, document, and utilize an energy control procedure for each piece of equipment.  These energy control procedures would include the following:      a. A specific statement of the intended use of the procedure,   b. Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy,    c. Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them, and   d. Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.

29 CFR 1910.147(c)(7)(i): The training program of the employer did not include all the training elements listed under Items A-C of 29 CFR 1910.147(c)(7)(i):     Carstin Brands, Inc. had employees perform maintenance operations on equipment including but not limited to: saws, cnc machines, and sanders, and the employer did not provide lockout training to ensure that the purpose and function of the energy control program was understood by employees.  This training would include the following:      a. Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control,   b. Each affected employee shall be instructed in the purpose and use of the energy control procedure, and  c. All other employees whose work operations are or may be in an area where energy control procedures may be utilized, shall be instructed about the procedure, and about the prohibition relating to attempts to restart or reenergize machines or equipment which are locked out or tagged out.
