29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:   (a) On or about 5/7/2014 - at Building A, 115 Greensboro Ave., Tuscaloosa, AL, the job-made ladder between the second and third floors near the wall identified as unit 211 type C2A, did not extend at least 3 feet above the upper landing surface and a grasping device was not provided.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) On or about 5/7/14 - at Building A, 115 Greensboro Ave., Tuscaloosa, AL, employees were not protected from falling about 9-feet at an unprotected side or edge where the job-made ladder was located between the second and third floors near the frame marked unit 211 type C2A.  South Georgia Framers, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 911560, citation number 1, item number 2 and was affirmed as a final order on 07/09/2013, with respect to a workplace located at 2701 East Nifong Boulevard, Columbia, MO 65201.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  a) On or about 5/7/14 - at Building A, 115 Greensboro Ave., Tuscaloosa, AL, employees were not protected from falling about 9 feet when installing floor joists and working at the leading edge.   South Georgia Framers, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 682219, citation number 1, item number 3 and was affirmed as a final order on 12/11/2012, with respect to a workplace located at 350 Brampton Ave., Statesboro, GA 30458.
