29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  a) Eastside Outside: On or about May 8, 2014 and at times prior thereto, there were knockout openings on the Culter Hammer ETN Safety Switches for Air Purification, condensing units and ice machine unit.   b) Mechanical Room:  On or about May 8, 2014 and at times prior thereto, there was separation at the elbow from the wall with openings exposing energized wiring leading into the mechanical room.

The cord was plugged into the outlet which provides water for the bathroom and kitchen.   b) Mechanical Room: On or about May 8, 2014 and at times prior thereto, a power strip was suspended and connected to the Culligan water treatment system used as fixed wiring to provide power.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  a) Workshop: On or about May 8, 2014 and at times prior thereto, the flexible cord on Craftsman Dry Vac Model 113 had damaged insulation showing conductor wires at the base of the flexible hose where employees touch the cord.  The damaged insulation exposed employees to shock hazards when using as needed for the daily clean up tasks. The vacuum was listed for household use but was used in an industrial setting.  b) Workshop: On or about May 8, 2014 and at times prior thereto, the flexible cord on Craftsman Compressor Model 919 had damaged insulation showing conductor wires at the base of the flexible hose where employees touch the cord.  The damaged insulation exposed employees to shock hazards when using for the weekly tasks.
