29 CFR  1910.37(a)(3): Exit routes were not kept free and unobstructed.  Materials or equipment were placed, either permanently or temporarily, within the exit route.  Located in the casino receiving area, exit routes were not kept free and unobstructed. Materials including a rolling cart and a pallet with boxes including a Vitamix and Schaerer coffee machine were in front of the exit.

29 CFR  1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user.  The elements of a written respiratory protection program including medical evaluation and storage requirements were not established for employees who may voluntarily use an elastomeric half mask respirator with P100 organic vapor cartridges. Employees are exposed to respirator hazards.

29 CFR  1910.134(h)(2)(i):Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve.   Located in the maintenance shop, two elastomeric half facepiece respirators were stored improperly. One respirator was hanging off a hook by the strap and another was laying on a work bench. Employees are exposed to respirator hazards including respirator failure, malfunction, or degradation.
