29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access. (a) Mifflin County Junior High School, Lewistown Pennsylvania - An employee climbed the end frames and cross bracing to access the working area on the second buck of the welded frame mason's scaffold to grind out the mortar joints on or about May 7, 2014. No abatement certification or documentation required.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) Mifflin County Junior High School, Lewistown Pennsylvania - Employees worked on the roof of the school building without an authorized method of fall protection. The work involved grinding joints in the masonry façade the contractor had erected an un-flagged rope around the work area. This was not an authorized, effective, form of fall protection and employees were exposed to a fall of approximately 13 feet from the roof to the ground below. No abatement certification or documentation required.
