29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury.    (a)Perez Construction, LLC at 280 Dusk Place, Erie, CO 80516: On and before April 25, 2014, the exposing employer did not ensure that a superintendent tasked in overseeing the jobsite was instructed in the recognition and avoidance of unsafe conditions associated with the placement of structural steel beam at a new residential home under construction. Employees were engaged in manually placing a 37 foot steel floor beam weighing 646lbs onto a residential foundation without adequate mechanical assistance. While attempting to manually set the beam into the pockets, one end of the beam slipped out and struck an employee working off of a ladder that was manually assisting in the setting of the structural beam. As a result of being struck-by the beam the employee was hospitalized. This condition exposed employees to a struck-by and crushing hazards.
