(a)Perez Construction, LLC at 280 Dusk Place, Erie, CO 80516: On and before April 25, 2014, the exposing employer did not ensure that a superintendent tasked in overseeing the jobsite was instructed in the recognition and avoidance of unsafe conditions associated with the placement of structural steel beam at a new residential home under construction. Employees were engaged in manually placing a 37 foot steel floor beam weighing 646lbs onto a residential foundation without adequate mechanical assistance. While attempting to manually set the beam into the pockets, one end of the beam slipped out and struck an employee working off of a ladder that was manually assisting in the setting of the structural beam. As a result of being struck-by the beam the employee was hospitalized. This condition exposed employees to a struck-by and crushing hazards.
