29 CFR 1910.176(b): Storage of material created a hazard: a) On or about May 13, 2014, aluminum chips and swarf from sawing operations were collected from various productions areas and stored in open top dumpsters located in Facility #2 Billet House and 7 inch Press Area. The dumpsters used for collecting aluminum chips and swarf were not covered or protected from ignition sources. Aluminum fires present unique hazards to nearby employees and fire fighters.

29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinders were designed to accept caps, were not always in place, hand-tight, except when cylinders were in use or connected for use: a) On or about October 9, 2014, a compressed gas cylinder in the Facility #2, Billet House, next to the aluminum chip and swarf dumpster, did not have the valve protection cap in place.
