29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. (a) Roof, new residential construction located on Lot #59 Bryce Creek Drive, Newnan, GA 30265: Employees were not protected from eye injuries while engaged in framing and sheathing work with pneumatic Hitachi (Model-NR83A2) Framing Nail Guns as these tools were operated without safety glasses or other eye protection. Jose Alvardo was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 901109, citation number 1, item number 1 and was affirmed as a final order on 06/28/2013, with respect to a workplace located at 2965 Vine Ridge Drive, Powder Springs, GA 30127.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) Roof, new residential construction located on Lot #59 Bryce Creek Drive, Newnan, GA 30265: An employee was exposed to falls over 25-feet while installing 1/2-inch plywood sheathing on a 7/12 pitched hip roof with no fall protection system in place. Jose Alvardo was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 902850, citation number 1, item number 1 and was affirmed as a final order on 09/10/2013, with respect to a workplace located at 4184 Lilycrest Way, Powder Springs, GA.
