29 CFR 1915.73(c): When employees were working around open hatches not protected by coamings to a height of 24 inches, or around other large openings, the edge of an opening was not guarded in the working area to a height of 36 to 42 inches. a.) Blue Ridge Tank Cleaning, Inc., Colonna's Shipyard, Barge DS55: On or about 05/12/14, employees on the deck of the barge were working around an open hatch exposing them to a potential fall of 19 feet.

29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2). In the Alternative 29 CFR 1915.77(c): When employees were working aloft, or elsewhere at elevations more than 5 feet above a solid surface, either scaffolds or a sloping ladder, meeting the requirements of Subpart E- Scaffolds, Ladders, and Other Working Surfaces, were not provided to afford safe footing, and employees were not protected with safety belts or lifelines meeting the requirements of 29 CFR 1915.159 and 1915.160. a.) Blue Ridge Tank Cleaning, Inc., Colonna's Shipyard, Barge DS55: On or about 05/12/14, fall protection was not provided for an employee working along the edge of the deck outside of the guardrails, thereby exposing the employee to a fall of approximately 19' to the pier.
