29 CFR 1915.12(d)(5): The employer did not certify that the training required by paragraphs (d)(1) through (d)(4) of this section had been accomplished. a) Colonna's Shipyard, Vessel ATC 200, Cargo, Overflow, Phenol Tanks and Voids: On or about 02/09/2016, the employer did not certify that training was provided to employees prior to entry in confined and enclosed spaces and other dangerous atmospheres in shipyard employment. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
