29 CFR 1910.22(c): Covers and/or guardrails were not provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc. a) For the Western International facility as observed on or about 05/19/2014, employees were exposed to trip hazards from a two foot wide drainage ditch/trough in the path of travel and two pipes that extended over the ditch. In order to walk through the area, employees had to step over the pipes and ditch/trough which was positioned in the path of travel.
