On 18 July 2018 through 20 July 2018, employees were walking and working within the gas acetylene plant and yard without the use of eye protection, exposing the employees to the hazard of being struck in the eye(s) by flying particles or debris.

29 CFR 1910.1001(d)(1)(i): The employer did not determine employee exposures from breathing zone air samples that were representative of the 8-hour TWA and/or 30-minute short-term exposures of each employee: On 18 July 2018 through 20 July 2018, employees were walking and working in the gas acetylene plant and yard without the employer performing a site characterization to determine if asbestos containing materials or presumed asbestos containing material were present, exposing the employees to the hazards associated with asbestos.
