29 CFR 1910.217(c)(3)(iii)(e): Safety distance(s)(Ds) from the presence sensing field to the point of operation of mechanical power press(es) were not greater than the distance determined by the formula specified at 29 CFR 1910.217(c)(3)(iii)(e)    a)  On May 20, 2014, the employer did not assure an employee operating the Niagara 150 Ton part revolution mechanical power press (Serial # 49007) was protected from amputation injuries.  The light curtains used to protect employees from the point of operation while feeding stock were set too close for the stopping time of the press ram.    b)  On July 3, 2014, the employer did not assure an employee operating the Minster 500 Ton part revolution mechanical power press (Serial # SEA-500-01479) was protected from amputation injuries.  The light curtains used to protect employees from the point of operation while feeding stock were set too close for the stopping time of the press ram.

29 CFR 1910.217(e)(1)(i): A program of periodic and regular inspections of mechanical power press(es) was not established and followed to ensure that all parts, auxiliary equipment and safeguards were in a safe operating condition and adjustment:      a)      On or about May 20, 2014, the employer did not ensure that all parts, auxiliary equipment, and safeguards of the part revolution presses were routinely inspected for safe operating condition and adjustment. The light curtains were adjusted such that the operators hands could reach the point-of-operation during the hazardous portion of the cycle.

29 CFR 1910.217(e)(3): Original and continuing competence of personnel maintaining mechanical power press(es) was not insured:  a)      On or about May 20, 2014, the employer did not ensure that the employees maintaining the power presses and the auxiliary safety equipment were adequately trained.  The light curtains on the part revolution power presses were adjusted such that the operators hands could reach the point-of-operation during the hazardous portion of the cycle.
