29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Jobsite: Second floor.  Employees were exposed to slips, trips, and falls while accessing/egressing the scissor lift platform where access/egress had not been provided and the crew was climbing over the platform guard rails.

Employees were exposed to falls of approximately 16 feet to the ground below where approximately 350 feet of wire rope guard rail had not been flagged.

Employees were exposed to falling approximately 16 feet onto the concrete floor below through two (2) unguarded floor holes measuring approximately 4 feet by 8 feet.

29 CFR 1926.502(b)(4): Components used to construct the guardrail system had not been designed for the purpose:  Jobsite: North and West sides, 2nd floor.  Employees were exposed to falls of approximately 16 feet to the ground below while working/walking on the floor where one-hole, electrical metallic tubing (EMT) clips, which had not been so designed by the manufacturer, or tested and/or listed by an approved laboratory to be used as applied, were used to construct the wire rope perimeter guardrail system as fasteners at the columns.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  Jobsite: West (Norwood Street) side of building, 2nd floor.  Employees were exposed to falls of approximately 16 feet onto the concrete floor below while working/walking in close proximity to a 4 feet gap in the decking that ran approximately 300 feet.   Pinnacle Consulting, Inc. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 1926.501(b)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER  912114, CITATION NUMBER 1, ITEM 3, ISSUED ON 6/25/2013 WITH A FINAL ORDER DATE OF 7/18/2013.
