29 CFR 1926.454(b)(2): The employer did not have each employee who was involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the correct procedures for erecting, disassembling, moving, operating, repairing, inspecting, and maintaining the type of scaffold in question:  On or about August 3, 2016, employees were exposed to a 30 foot fall hazard while performing work activities from a scissor lift that had a damaged safety gate chain.  The employer did not ensure that each employee performing inspections had been trained by a competent person.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:  On or about August 3, 2016, employees were exposed to fall hazards greater than 30 feet and the employer did not ensure employees were protected from a fall by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  Pinnacle Consulting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.760(a)(1), which was contained in OSHA inspection number 912114, citation number 1, item number 4, and was affirmed as a final order on July 24, 2013, with respect to a workplace located at 14130 Beach Blvd. Jacksonville, FL 32256.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
