29 CFR 1926.651(i)(3): Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures. a) Jobsite: Residential parking lot - On or about 5/22/14, a support system for an undermined section of pavement was not used to protect employees from collapse hazards. Employees were installing a concrete box inside the excavation measuring approximately 10 feet in length, 8 feet in width, and 5 feet in depth.

Employees were installing a rain run-off concrete block.
