29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  a) 1124 Day Street, Philadelphia, PA - Employees were working inside an excavation which was approximately 8 feet long by 8 feet wide by 5.5 feet deep without having any means of egress, thus exposing employees to trip/falls and cave-in hazards. This violation was observed on or about June 28, 2014.

29 CFR 1926.651(i)(3): The pavement had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures:  a) 1124 Day Street, Philadelphia, PA - Employees were working inside an excavation which was approximately 8 feet long by 8 feet wide by 5.5 feet deep in which the pavement was undermined and a support system was not installed to protect employees from a possible collapse. This violation exposed employees to struck-by hazards. Observed on or about June 28, 2014.

Protection by placing and keeping such materials or equipment at least 2 feet from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary was not provided:  a) 1124 Day Street, Philadelphia, PA - Employees were working inside an excavation which was approximately 8 feet long by 8 feet wide by 5.5 feet deep in which the spoils pile was not placed or kept at least 2 feet from the entrance of the excavation nor were any retaining devices used. This violation exposed employees to cave-in/struck-by hazards. Observed on or about June 28, 2014.
