29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    (a) Roof, new residential construction located at 2679 Hughes Street, Smyrna, GA 30080: Employees were exposed to eye injuries while operating pneumatic Hitachi (Model NV45AB2) Coil Roofing Nail Guns without eye protection. Employees firing 1-1/4-inch nails at 120 PSI were operating these tools without safety glasses, goggles or other face protection.

29 CFR 1926.300(b)(2): Moving part(s) of equipment exposed to contact by employees or which otherwise created a hazard, were not guarded:    (a) Front side, new residential construction located at 2679 Hughes Street, Smyrna, GA 30080: Employees were exposed to amputation hazards while using a Jenny 8-Gallon Air Compressor without the belt guard in place. Employees operating this equipment were exposed to moving belts and pulleys located near the on/off controls.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     (a) Roof, new residential construction located at 2679 Hughes Street, Smyrna, GA 30080: Employees were exposed to falls while engaged in roofing activities on a 7/12 pitched roof at eave heights of approximately 12'4" and 19' to the ground level as there was no fall protection system in place.

29 CFR 1926.552(a)(1): The employer did not comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators.       (a) North side, new residential construction located at 2679 Hughes Street, Smyrna, GA 30080: Employer allowed employees to use the TranzSporter (Model LH200) Platform Hoist as a ladder to access the roof. The Manufacturer's General safety instructions state that this hoist should never be used as a ladder. Employees using this material hoist equipment to access the roof were exposed to falls of approximately 36-inches as the bottom of the hoist frame was obstructed by the platform hoist and the motor mounts.
