29 CFR 1910.95(g)(7)(ii): Where the annual audiogram showed that an employee had suffered a standard threshold shift, the employer did not obtain a retest within 30 days. (a) Facility: On or about May 30, 2014, and at times prior to, the employer failed to retest an employee that had suffered a recordable standard threshold shift. On July 15, 2014, an employee working on and around plastic extrusion machines was exposed to continuous noise at a level of 66.7% of the permissible exposure limit (PEL) dose. The equivalent dBA for 66.7% of the dose is approximately 87.0 dBA calculated as the 8 hour time weighted sound pressure level. The monitoring was conducted for 429 minutes. Zero exposure was assumed for the remaining 51 minutes of the day.

29 CFR 1910.95(g)(8)(ii)(B): Unless a physician determines that the standard threshold shift is not work-related or aggravated by occupational noise exposure, the employer did not ensure, when a standard threshold shift occurs, that for employees already using hearing protectors, employees be refitted and retrained in the use of hearing protectors and provided with hearing protectors offering greater attenuation if necessary: (a) Facility - On or about May 29, 2014, and at times prior to, an employee who had suffered a recordable standard threshold shift, who was already using hearing protectors, was not refitted for hearing protectors. A physician had not determined that the threshold shift was not work-related. On July 15, 2014, an employee working on and around plastic extrusion machines was exposed to continuous noise at a level of 66.7% of the permissible exposure limit (PEL) dose. The equivalent dBa for 66.7% of the dose is approximately 87.0 dBA calculated as the 8 hour time weighted sound pressure level. The monitoring was conducted for 429 minutes. Zero exposure was assumed for the remaining 51 minutes of the day.

29 CFR 1904.10(a): Where an employee's hearing test (audiogram) revealed that the employee had experienced a work-related Standard Threshold Shift (STS) in hearing in one or both ears, and the employee's total hearing level was 25 decibels (dB) or more above audiometric zero (averaged at 2000, 3000, and 4000 Hz) in the same ear(s) as the STS, the employer did not record the case on the OSHA 300 Log. (a) Facility - On or about May 29, 2014, and at times prior to, the employer failed to record a standard threshold shift that averaged greater than 10 decibels with a total hearing level of 25 decibels or greater. On July 15, 2014, an employee working on and around plastic extrusion machines was exposed to continuous noise at a level of 66.7% of the permissible exposure limit (PEL) dose. The equivalent dBa for 66.7% of the dose is approximately 87.0 dBA calculated as the 8 hour time weighted sound pressure level. The monitoring was conducted for 429 minutes. Zero exposure was assumed for the remaining 51 minutes of the day.
