On or about June 6, 2018, Production and Warehouse Areas:  Employees were observed operating Nissan and Toyota propane-powered forklift vehicles throughout the facility without wearing the seatbelts provided on the equipment.  Powered industrial trucks were used to carry loads including rolls and plastic sheet, weighing up to 2,000 lbs.

Extruding Department, Line 1:  On or about June 6, 2018 and times prior thereto, the SMI light curtain provided at the finish end of the extruding line was not operational.  The electrical power cord had sustained damage during the movement of equipment.  No pre-shift inspection had been done to ensure that the presence sensing device was operational.
