29 CFR 1926.651(a): All surface encumbrances that were located so as to create a hazard to employees were not removed or supported, as necessary, to safeguard employees. a) Jobsite: South southwest corner of site by the headworks building - On or about 5/27/14, the footing above the west wall of the excavation was not supported as necessary to safeguard employees working inside the excavation installing 36 inch ductile pipe. The footing fell approximately 20 minutes after the employees were removed from the excavation when the west wall collapsed severely injuring two employees.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section. a) Jobsite: South southwest corner of site by the headworkds building - On or about 5/27/14, a protective system was not used to protect employees from cave-in hazards while installing a 36 inch ductile pipe inside the excavation measuring approximately 20 feet in length (north to south), 8 feet in width (west to east), and with a depth sloping from 10 feet to 3 1/2 feet (north to south). Pact II was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 949246, citation number 1, item number 1, and was affirmed as a final order on 1/2/14, with respect to a workplace located at 230-246 Parkway Drive, Orwigsburg, PA 17961.
