29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Note:Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.  a.)    Salem Veteran Administrations Medical Center, Building 13 Chiller Plant, Second Floor: On or about 05/28/14, a guardrail system was not provided along the edge of the second level floor therefore employees were potentially exposed to an approximate 20' fall.   b.)    Salem Veteran Administrations Medical Center, Building 13 Chiller Plant: On or about 05/24/14, a guardrail system was not provided for employees working on top of a water tank to add chemicals, as a result employees were exposed to an 8' 5" fall.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Salem Veteran Administrations Medical Center, Building 10: On or about 05/29/14, the exit route was not kept clear in that a sump pump was placed directly outside the exit door, as a result employees were potentially exposed to a trip hazard.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Salem Veteran Administrations Medical Center, Building 7, Attic: On or about 05/29/14, a guard was not enclosed around the vertical belt and pulleys to the HVAC unit as a result employees were exposed to an in-running nip point hazard. Salem Veteran Administrations Medical Center, Building 4 Food Service: On or about 05/29/14, the path to ground on the portable floor buffer was not permanent and continuous in that the ground prong was missing from the male attachment plug, thereby exposing employee(s) to a potential shock hazard.
