29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet or more above a lower level was protected from falling by one or more of the following:  guardrail systems, safety net systems, or personal fall protection systems:  (a) Veterans Affairs Medical Center at 1055 Clermont Street, Denver, CO 80202: On and before August 14, 2017 the employer did not ensure employees were protected from falls in excess of 4 feet when working on steam valves. Employees were assigned to turn-on and to turn-off steam valves located in the sub-basement compressor room and hot water heater room, as well in the 6th floor mechanical room.  Employees used 8 and 10 foot portable ladders to access the steam valves, but stepped off of the ladders and climbed onto the piping to reach the steam valves. This condition exposed employees to a fall hazard greater than 4 feet.

29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee was protected from falling through any hole (including skylights) that was 4 feet or more above a lower level by one or more of the following: covers, guardrail systems, travel restraint systems, or personal fall arrest systems:  (a) Veterans Affairs Medical Center at 1055 Clermont Street, Denver, CO 80202:  On and before August 14, 2017, the employer did not ensure that employees were protected from falling through a hole located in an access panel on the A wing on the first floor next to stair # 9.  Employees accessed this panel to shut off the domestic supply water valve that controlled water to the A wing.  This condition exposed employees working inside of the access panel to a fall hazard greater than 4 feet.

29 CFR 1910.1200(g)(1): Employers did not have a safety data sheet in the workplace for each hazardous chemical which they use:  (a) Veterans Affairs Medical Center, Denver, Colorado 80202:  On and before August 14, 2017 the employer did not maintain a safety data sheet for each hazardous chemical and lubricant, flammable liquid, and combustible liquid stored in the medical center.  The hazard communication program contained material safety data sheets for these chemicals, but did not include the safety data sheets.  This condition exposed employees to chemical irritations to eyes, respiratory, and skin due to employee lack of knowledge regarding chemical hazards.
