29 CFR 1926.850(c): All electric service lines were not shut off, capped, and/or otherwise controlled, outside the building line before demolition work was started:    a) 4100 Parkside Ave, Philadelphia, PA - The employer did not ensure that the electrical service lines had been de-energized prior to beginning the demolition process thus exposing the employees to electrocution/shock hazards. This condition was observed on or about May 28,2014.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.850(a): Prior to permitting employees to start demolition operations, an engineering survey was not made by a competent person of the structure to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure, including adjacent structures where employees may be exposed. The employer did not have in writing evidence that such a survey was performed:    a) 4100 Parkside Ave, Philadelphia, PA - The employer did not perform an engineering survey to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure, including adjacent structures where employees are exposed to hazards. This hazard exposed the employees to struck-by and caught in-between hazards. This condition was observed on May 28, 2014.
